For Spring 2010, we will have registration at the Malverne Village Hall basement.   Please use the back side entrance.  Malverne Village Hall is located at 99 Church St.  Registration will be held on:

• Wednesday / December 30th  / 7:00 pm - 9:00 pm
• Wednesday / January 6th / 7:00 pm - 9:00 pm
• Thursday/  January 7th / 7:00 pm - 9:00 pm

Teams for both boys and girls will be entered into the Nassau County Police Activity League (PAL) on a "best efforts" basis, meaning if after registration we have enough players and coaches for a particular grade level then we will enter a team for that grade level.  If we do not have a sufficient number of players and/or coaches to field a team at a particular grade level then we will return all fees paid at registration for that grade level.

To Register please provide the following:

(1) Completed PAL membership application.

(2) Completed US Lacrosse membership.  US Lacrosse membership can be obtained by filling out the attached form or by registering online for US Lacrosse membership. Fee is $25 annually. Please bring your membership number if you register online.  Otherwise, please make a check payable to 'US Lacrosse' for $25 and bring a copy of the attached form.

(3) Registration fee as follows:

• 3rd-8th grade                                                     $75
• 1st-2nd grade (new players)                                 $45
• 1st-2nd grade having uniforms                               $30

Make check payable to 'Malverne Lax'

(4) Copy of your child's birth certificate (new players only)

(5) Uniform fee of $60 for 3rd-8th grade (new players only). Check should be made out to 'Malverne Lax'.

(6) Parental consent/liability waiver(to be filled out at registration)
